Mile 22 is a web application that optimizes the day-to-day work of new and existing NDPP providers so they can serve more people.

Fewer than 1% of the 96 million eligible Americans enrolls in a CDC-recognized diabetes prevention program (NDPP) each year. Low enrollment is not due to lack of interest, but rather lack of operational infrastructure to support referral pipelines, support health coaches relieve SDOH barriers, and reduce time spent on paperwork.

The Mile 22 platform gives NDPP health coaches the infrastructure to launch and run programs without the administrative burden that shuts most of them down. For a monthly subscription, NDPP provider organizations and hubs can access:

  • Streamlined referral management

  • Standardized participant enrollment

  • Automated CDC reporting

  • Population health insights

  • Ongoing expert support

The product name comes from marathon culture: mile 22 is where most runners hit the wall and want to quit. It is also the mile that separates those who finish from those who don't. Mile 22 is also literal: the CDC’s National Diabetes Prevention Program Program (DPP) runs for 22 sessions over 12 months.
